Request your Disciplinary Record
George Washington University students are able to view their record maintained by the Office of Student Rights & Responsibilities and the Office of Academic Integrity. If you would like to know what information about you, if any, is maintained by our offices please fill out the form below. A representative from the Office of Student Rights & Responsibilities or the Office of Academic Integrity will contact you in a timely manner with the information you requested.
Additionally, you may also request to review information maintained in your administrative or disciplinary record by contacting our office at (202) 994-6757 or via email at email@example.com and setting up a meeting with a member of our staff. Administrative and disciplinary records are not immediately available to students because the records must be redacted, which means information relating to you and other individuals involved along with any other confidential information must be removed. Upon request, the Office of Student Rights & Responsibilities and the Office of Academic Integrity will provide students with copies of redacted letters and case materials maintained in the student's file.
Please note that multiple fields are required. Incomplete forms will not be accepted.
